BPH vs. Overactive Bladder: How to Tell the Difference

However, there are some distinct differences between BPH vs. OAB, which may help you identify which is most likely causing your symptoms. In a general sense, some of the primary differences in BPH vs. overactive bladder syndrome are that BPH involves the prostate primarily, while OAB impacts the bladder. 

Additionally, the symptoms of BPH are typically more obstructive and restrictive — like difficulty urinating — vs. OAB symptoms, which are usually characterized by urgency and frequent urination. Again, however, there is certainly some overlap and/or similarity between the two. Does BPH cause bladder problems?  

BPH can certainly cause urinary symptoms, though BPH is actually a benign condition affecting the prostate gland. In men with BPH, or benign prostatic hyperplasia, the prostate becomes enlarged. As a result, the extra prostatic tissue can place undue pressure on the urethra and bladder, leading to noticeable changes in urinary habits and more. Some of the most common symptoms of BPH related to the bladder and urination include:  

  • Difficulty voiding 
  • Trouble starting urination (or stopping) 
  • Burning with urination 
  • Frequent urination at night 
  • Pushing or straining to void 
  • Feeling of incomplete bladder emptying 
  • Dribbling or spraying urinary stream 

 

How do I know if I have overactive bladder vs. BPH?  

Both overactive bladder and BPH can lead to changes in urination, though they are usually fairly distinct, depending on which condition is responsible. While everyone's case is unique, the most common OAB symptoms usually include things like:  

  • Frequent urination 
  • Urgency 
  • Incontinence 
  • Can't stop urinary stream once it starts 
  • Sudden sense of needing to void 
  • Bladder spasms 

BPH, on the other hand, usually involves more restrictive or obstructive urinary symptoms, like dribbling urinary stream and the need to push to urinate.
